We have also included a hard coded example (as opposed to written dynamically using document.write() calls like the above date elements) in case you already have a form set up and just want to add these date elements to it. The way we got the hard coded HTML that is used to display was by putting this file online on our web server. The we accessed it through it's URL and chose "View Source" in Netscape. Then we copied the code from that window. So if you need hard coded form elements with different specifications (e.g. not abbreviated month names or fewer years in the year menu), then you can put this document on the web with your new specs and grab the hard coded HTML from the View Source window.

Notice also that the code works independently for multiple forms on the same page.
